The staging method proposed by Braak and Braak allows the objectiveand reliable assessment of Alzheimer-related neurofibrillarypathology. Originally the method was designed for 100-mu m thicksections. However, the use of thick sections proved to presentdifficulties in a routine neuropathology laboratory. In order toadapt the staging method for thin paraffin-embedded sections, weperformed an inter-and intrarater study analysing the reliability ofthe staging method in thin sections. Statistical analysis of the dataprovided by six independent examiners in two rating sessions revealkappa values of 0.6-0.8 for both the interrater and the intraraterreliability. The average rate of mistake of the examiners was rarelybigger than a half stage. We conclude that the adapted staging methodin thin sections is strongly reliable and we recommend it for stagingpurposes in institutions where the preparation of thick sectionswould be difficult.
