This study measured brain atrophy in patients with idiopathicParkinson's disease and diffuse Lewy body disease, all of whom hadequivalent loss of midbrain dopaminergic neurons and absence ofAlzheimer's disease, Characteristic patterns of volume loss werefound throughout the brain? depending on the age of onset andclinical signs. An equivalent loss of medial temporal lobe structuresoccurred in all parkinsonian patients. This atrophy was similar inmagnitude to that seen in Alzheimer's disease and is likely to be theanatomical substrate for the memory deficits found in each of thesepatient groups, Frontal lobe atrophy was a feature of both late-onsetParkinson's disease (mild atrophy) and diffuse Lewy body disease(significant atrophy) groups, with all cases analyzed havingdementia. Atrophy of frontal lobes correlated with the duration ofmotor symptoms in these patients and may suggest an associationbetween dopaminergic deafferentation. Frontal atrophy and dementia.
