The number of missed cases is even higher among patients with minor strokes. This work examined whether stroke training for paramedics, prehospital use of the National Institutes of Health Stroke Scale (NIHSS) on a mobile app, and standardized communication with stroke physicians could improve stroke recognition. Methods included a stepped wedge cluster randomized trial in the Oslo ambulance service targeting patients with suspected stroke, with additional analyses of those presenting with a low NIHSS score of 0–2. The intervention did not increase diagnostic accuracy overall. The NIHSS scores in the intervention group were significantly lower, indicating improved detection of subtle strokes. Prehospital time increased, likely due to the pandemic, while in-hospital time decreased through more efficient communication. Among very mild strokes there was significant overlap in symptoms between stroke and non-stroke patients, and NIHSS showed high sensitivity but low specificity. Overall, the intervention, including prehospital NIHSS, proved feasible and valuable for identification of all strokes."
