This thesis includes three qualitative studies using reflexive thematic analysis. Study I explored the experiences of multidisciplinary healthcare professionals in supporting adolescents transitioning off treatment. While recognizing families’ challenges and support needs, their efforts were often constrained by time and system barriers. Study II examined adolescent survivors and their parents’ transition experiences, revealing needs for clearer information, better preparation, and stronger psychosocial support. Study III aimed to address these care gaps by co-creating a nurse-led transition intervention with involved stakeholders. The intervention included two nurse consultations—before and after treatment—and a digital meeting with the family’s local network to ensure continuity of care. Feasibility testing with ten adolescents showed the approach was acceptable and well-received.. - This thesis investigates the transition from active cancer treatment to follow-up care for adolescents, focusing on the psychosocial challenges and support needs of both patients and their families. Through three qualitative studies, it examines healthcare professionals' experiences, the perspectives of adolescent survivors and their parents, and the development of a nurse-led intervention to address care gaps. The findings highlight the importance of clear communication, preparation, and multidisciplinary support to ensure a smooth transition and continuity of care.
