Residents of nursing and residential homes are often prescribedmedication for physical and mental ill-health with resultantpolypharmacy and the possibility of iatrogenic disorders. Sometimesdrugs are prescribed inappropriately and a number of studies havehighlighted the overuse of psychotropic drugs. Legislation in the USAhas been effective in controlling their use in that neurolepticprescriptions for the treatment of behavioural disturbances have beensignificantly reduced and non-pharmacological strategies aimed atameliorating behavioural disturbance have been proposed. Studiessuggest that pharmacists have a role to play iii the appropriatemanagement of medicines in elderly residents; they can improvepatient care and be cost-effective. Identification of methods bywhich to improve awareness of inappropriate medication prescriptionand to avoid certain types of drugs is required. This reviewcritically evaluates medication use in this population with specificreference to psychotropics and aims to raise awareness of the issue.(C) 1998 John Wiley & Sons, Ltd.
