Mania in late life is often complicated by comorbid medicalconditions, medication intolerance, and inadequate medicationresponse. Lithium remains the primary medication in the acute andprophylactic treatment of mania in the elderly. However, certainanticonvulsants (valproic acid and carbamazepine) have a number ofadvantages, including a more tolerable side effect profile andincreased efficacy in subgroups of patients, such as patients withsecondary mania due to neurological and other medical disorders.Future research is needed to focus on the double-blind trials ofsomatic treatments, including ECT, in the elderly.
