Abstract Senile dementia of the Alzheimer type (SDAT) is characterized by progressive memory loss, decline in ability to perform routine taskes, impaired judgement and attention, disorientation, and difficulty in learning. Understandable concern has been raised about the safety of persons with SDAT who drive motor vehicles. However, blanket recommandations to revoke driving privileges following the diagnosis of SDAT may be inappropriate. Not only is there the possibility of misdiagnosis, particulary in the early stages of dementia when individuals are most apt to be driving, but also driving performance in at least some persons in the milder stages of SDAT still may be satisfactory. Few studies, however, have attempted to correlate driving skills with the level of dementia severity.Acutal driving performance in SDAT has seldom been assessed. We conducted a pilot study to determine the feasibility of assessing skills by means an in-car, on-the-road examination in persons with SDAT and to determine whether driving compentency can be maintained in those who are only mildly affected. We also explored the predictive ability of predriving clinical tests for driving performance in SDAT.
